Abstract: A study of brain stem auditory-evoked potentials (BAEPs) and visual-evoked potentials (VEPs) was done on 25 newly diagnosed patients with leprosy whose diagnosis was confirmed by skin biopsy. The results were compared with 25 age- and sex-matched healthy controls. In BAEPs the important observations were the prolonged latency of wave V in 13 (52%), delayed interpeak latency (IPL) of wave I-III in 5 (20%) cases, of wave III-V in 12/25 (40%), suggesting a conduction abnormality of the VIII cranial nerve in its peripheral part, in its nucleus and in its connection in the brain stem. In VEPs, a delayed peak latency of major positive potential (P100) was seen in 20 cases (80%; 11/13, 84.6% TT; 7/10, 70% LL; 2/2, 100% BL), suggestive of subclinical optic nerve involvement. The BAEPs and VEPs were both abnormal in 10 cases (40%; 3/13, 23% TT; 5/10, 50% LL; 2/2, 100% BL). Conduction abnormalities of the central nervous system (CNS) were observed more frequently in lepromatous leprosy, as in other forms of peripheral neuropathy such as hereditary motor sensory neuropathy type I (HMSN I). There is a fair possibility of similar multiple demyelinating lesions in the CNS also, as is seen in leprous peripheral neuropathy. This hypothesis requires further strengthening by an extensive study of multimodality evoked potentials with magnetic resonance imaging in the patients. Histopathological and immunofluorescent studies of autopsy material of the brain can also contribute significantly to solve the dilemma. (AU)^ien. Lepr;66(3):340-347, Sept. 1998. tab. Abstract: The changes in nerve function tests in 297 new leprosy patients over an average period of 30 months were measured. The impact of type 1 reactions (T1R) on sensory and voluntary muscle function was measured by standard tests. Sensory function was improved in patients with single episodes of cutaneous T1R, but not improved in patients with neural T1R or with multiple episodes of either kind of T1R. Patients over 40 years of age improved less than younger patients, and patients admitted for treatment of T1R improved more than those treated as outpatients. These data point to a need to find better regimens for the treatment of nerve damage in T1R. (AU)^ien. Abstract: In Brazil, there is little information about the clinical and epidemiological characteristics of paucibacillary, single skin lesion leprosy patients (SSL-PB). Only recently has the official notification system distinguished leprosy patients with a single lesion as a clinical entity, for whom the single-dose ROM (rifampin, ofloxacin and minocycline) regimen has been recommended. In this paper, we describe the baseline clinical features and the immunological background of a multicenter cohort of SSL-PB leprosy cases enrolled between December 1997-1998. Patients were recruited at health centers located in the following regions: Southeast = Rio de Janeiro; North = Amazon and Rondônia states and Center-West = Goiás state. Eligible cases were newly detected, untreated single-lesion leprosy patients without thickened nerve involvement, and were assessed by clinical, bacilloscopic and histopathological exams. The Mitsuda skin test and anti-PGL-I serology (ELISA) were also performed. Of the 299 SSL-PB leprosy patients, 259 (86.6%) fulfilled the criteria for single-dose ROM intervention. Our results showed that patients recruited from different sites had similar features, considering the clinical and immunological profiles. There was a predominance of adults (mean age 32.4; S.D. = 16.0), and a BCG scar was detected in 76.7% of the children (< or = 15 years old). Only 7 cases were diagnosed as the multibacillary type, representing less than 3% of the patients being misclassified. Our data indicate that in Brazil SSL-PB case ascertainment based on clinical and bacilloscopic criteria can be accurately defined under a routine control program; 75.0% of SSL-PB cases were Mitsuda positive (> or = 5 mm) and seropositivity for anti-PGL-I was detected in 17.3% of the patients. These data are compatible with effective cell-mediated immunity and low bacillary load, suggesting favorable clinical outcomes for most SSL-PB participants of this cohort. (AU)^ien. Abstract: Os AA. consideram a posição da lepra dimorfa ("borderline") na classificação, baseados em 28 casos e nos seguintes elementos: características clínicas, bacterioscopia, histologia, prova lepromínica, freqüência e origem. Julgam que a lepra dimorfa é apenas uma fase de transição, que raramente ocorre no curso da moléstia ou por influência da terapêutica. Em alguns doentes, comporta-se de modo semelhante à lepra tuberculóide em reação, regredindo após alguns meses de tratamento, enquento em outros, exige até 3 ou 4 anos para sua evolução. Como a lepra evolui em "câmara lenta", esta fase persiste por tempo maior ou menor, mas constituiria apenas um fato de passagem entre uma forma e outra. Sendo assim e pela sua raridade, pensam que o grupo dimorfo deve ser exclu¡do da classificação; está, além de ser científica, deve possuir também cunho prático, abrangendo, de preferência, as formas comuns da molèstia e nâo raras ou os aspectos transicionais. Abstract: 1) - A relação masculino-feminino nos doentes de lepra de Uruguaiana, foi de 1: 1,06 e na população ‚ de 1: 1; discreta elevação nos femininos. 2)- Os brancos contaminaram-se 5,1 mais que os mistos e 2,2 que os pretos, nos respectivos coeficientes. 3)- Maior coeficiente de incidência nos viúvos e o mais baixo em menores de 15 anos. 4)- O sexo feminino foi o mais acometido (52 para 49), embora as mulheres fizessem mais formas de resistência. (31 para 25 dos homens). 5)- Os estrangeiros apresentaram um coeficiente de incidência 5 vêzes maior do que os nacionais, porém tiveram 65% de formas de resistência. 6)- Os estrangeiros contribuiram com 17% dos casos de endemia, sendo em maior número os uruguaios, seguidos dos argentinos. 7)- A incidência da leprose, em geral acompanhou os tipos de trabalho da comunidade. 8)- Descobriram-se 33 fontes de contágio e em 67,4% das vêzes permaneceram ignoradas. 9)- Quando conhecido o foco transmissor. 88% foi encontrado na própria fam¡lia e 76% no mesmo domicílio. Predominou o contágio de pais para filhos com 36,5%, seguido do mesmo entre irmãos (24,4%) e de tio para sobrinhos (12%). 10)- Sòmente o tipo lepromatoso transmitiu a doença de Hansen, apesar de terem existido 21, das formas clínicas com baciloscopia positiva. 11)- O tempo de incubação da moléstia em 31 comunicantes que enfermaram foi de 5,4 anos. 12)- O tempo de doença quando fichado, variou de 1 mês a 18 anos, com média de 3 anos e 3 mêses. 13)- A idade média quando se inicou a moléstia nos 101 leprosos, foi de 40 anos e 4 mêses, provando tratar-se de tipo de infecção tardia. Predominou no grupo etério de 30 a 39 anos. em 13 contatos que adoeceram após o fichamento foi de 34,5 anos. 14)- A incidência da leprose apresentou-se 21 vêzes mais elevada nos maiores de 15 anos que nos menores...(AU) ^ipt. Abstract: The author reports his work and other data about the correlation between tuberculosis and leprosy, considered under the immunoallergic, clinic and epidemiologic point of view. 1. Cosidering the immunoallergic correlation between tuberculosis and leprosy with all the data available, he says that in some materials there is an association between positive tuberculin and the positivity of lepromin. There are other elements that reduce the importance of the above mentioned association and of eventual consensibilization. New experiments should be done, especially out of urban areas and in children, in order to comprove that the paralellism of the tuberculin and lepromin curves depends of a relation of cause and effect and not of an association. 2. The researchs of Fernandez, Melson, Hadler and Ziti, and others, favor the hypothesis of transitory tuberculin sensibilization caused by the "M. leprae" and "m. lepraemurium". However, there are many data that reduce its importance. In case it come to be proved in other experiments it wil be difficult to apreciate correctly the correlation between the tuberculin and the lepromin test. When would the first influence to second of vice-versa? and in what degree? Even proved this cross sensibilization from "M. leprae" to "M. tuberculosis", the A. thinks that it would be of slight importance, without interfering largely in the specificity of the tuberculin test. 3. Tuberculosis influencing the clinical course of leprosy and vice versa: the author believes that the data are not enough to prove that tuberculosissnakes more favorable the clinical course of leprosy and vice versa. 4. Abstract: Após considerações gerais sobre a importância do problema do mal perfurante plantar como incapacidade f¡sica e sua gênese, o autor expõe as caracter¡sticas de um produto obtido do sangue de amimais jovens (Solcoseryl) e usado no tratamento desta complicação tardia de lepra. Em quatorze anos obtem doze curas com um m¡nimo de cinco e um máximo de 38 aplicações, e em dois casos obtem melhora evidente. Os resultados são ilustrados por fotografias anteriores e posteriores ao tratamento. Concluiu-se que, pela ação cicatrizante rápida, é de utilidade no tratamento, em regime ambulatorial, do mal perfurante plantar de origem leprótica. (AU)^ipt.
